We recommend that you read the Guide on cover and health information before you start filling in the health declaration.

You will have two options; you can either fill in the declaration online (A) or you can download it (B) and fill it in using Acrobat Reader. If you are using a tablet, you will only have the option to fill in the declaration online.

You cannot submit the declaration unless you have answered all questions.

Complete and submit the declaration by using electronic signature:

 

  • Fill in the form

    • If you do not have MitID 

    If you do not have MitID, please complete and sign the declaration electronically here.
